On behalf of our team, we would like to welcome you to Riverside Dental! We are elated that you have selected and trust our office to care for your dental needs, and we look forward to meeting and treating you!
YOUR FIRST VISIT
During your first visit, a comprehensive examination will be completed. Once we have worked together to develop the best plan for you, one of our experienced dental hygienists will begin your cleaning. Should a deeper cleaning be necessary, we may need you to come back. Your initial appointment at Riverside Dental will be 90 minutes long.
PREPARATION FOR YOUR FIRST VISIT
Please be prepared for your appointment by completing the following patient registration forms by clicking on the link below to our web patient web portal. Please note that the information you will submit will be encrypted for your protection and goes directly to our office. If you have dental insurance, be sure to provide all requested information before your first appointment. This will help us to best estimate your insurance coverage.
To complete a document, simply fill out the fields with the requested information. While most of the fields are optional, certain fields, marked by asterisks, must be completed.
